Five patterns account for almost every referred record or portfolio we see, across every component and every level.

It's a Diary, Not a Record

"Monday: busy shift, helped with breakfast, did the laundry." True, dated, and evidencing nothing. A learner record is self-reflective by definition โ€” what you did is the setup, not the answer.

The Wrong Technique Entirely

A beautifully organised folder of separate documents submitted where a reflective record was required โ€” or a personal reflective journal handed in where a collection of distinct pieces was asked for. It comes back before the content is even read.

Outcomes With Nothing Against Them

Fifteen entries, four of them covering the same outcome, and two outcomes with no evidence at all. Learners count entries; assessors count outcomes. It's the commonest gap and the easiest to close.

Written All at Once, at the End

Same tone, same pen, same level of detail across four months, with no development visible from the first entry to the last. Assessors and internal verifiers both notice, and it raises a question you don't want asked.

Somebody Is Identifiable

A first name and a diagnosis. A room number and an age. The setting's name in a document header you forgot to remove. Individually harmless, together identifying โ€” and a problem in its own right before grading even starts.

The Three Techniques

Learner Record or Portfolio โ€” Which One Does My Brief Mean?

These are QQI's own definitions, not our interpretations. Read them once and most of the confusion disappears โ€” including the reason your tutor and your brief seem to be describing different documents.

Technique 1

The Learner Record

Self-reported and self-reflective. You describe your own learning experiences, activities, responses and the skills you acquired. The key word is self โ€” it is your account of your own development, not a folder of things you produced.

Technique 2

The Portfolio or Collection of Work

A collection or selection of separate pieces produced over time, each demonstrating outcomes. Some pieces you generate yourself, others come from a brief your assessor sets. It is a set of artefacts, not a narrative.

Technique 3

The Assignment

An exercise completed in response to a brief with specific guidelines, usually one piece answering set questions. Shorter, more contained, and marked on whether you answered what was asked rather than on evidence gathered over time.

Not a technique

The Diary โ€” What Most People Submit

A dated account of what happened, with no reflection and no outcome named. It isn't a QQI assessment technique at all. It's usually the raw material for a learner record rather than the record itself.

A portfolio or collection of work is a collection of separate pieces produced over time, each demonstrating outcomes. One is a narrative about you; the other is a set of artefacts. They are different techniques, marked differently, and your component specifies which one it wants.
The brief. Assessment techniques are mapped to learning outcomes when a programme is validated, and the assessor works from that document. Tutors often use "portfolio" loosely to mean all evidence, which is where the confusion starts. Find the line on your brief or assessment plan naming the technique and its percentage โ€” that is what decides the shape of what you write.
No, and this is the most expensive misunderstanding in QQI assessment. A diary records what happened. A learner record is self-reflective โ€” what happened is only the setup. Three lines turn one into the other: what you noticed, what it told you, and what you would do differently. Most referred records are diaries that were never taken that final step.
A brief check before you write anything is โ‚ฌ25. Three entries as an early check is โ‚ฌ35, a full learner record โ‚ฌ59, and a full portfolio or collection of work โ‚ฌ65. A record with a second element starts at โ‚ฌ95, and a referral re-check is โ‚ฌ25. You get a firm figure before committing.
Your brief will usually set a number, but the count matters less than the coverage. All learning outcomes must be evidenced, so fifteen entries with two outcomes untouched fails where eight well-spread entries would pass. Count outcomes, not pages โ€” it takes fifteen minutes and it is the single most useful thing you can do before submitting.
You can, and it usually shows. Records written in one sitting have the same tone, the same level of detail and no visible development from the first entry to the last โ€” and assessors and internal verifiers both notice. More practically, you will have forgotten the specific moments that made the strongest evidence. Ten minutes a week beats a weekend in April.
What happened, what it told you, and what you would do differently. The first part is usually already written. The second is where the outcome is met, and the third is where a Merit turns into a Distinction. Entries where nothing was difficult evidence the least โ€” a moment you got something wrong and adjusted is worth more than a week where everything went smoothly.
Initials or role titles only, never the setting's name, and watch for combinations โ€” an age plus a condition plus a room number identifies someone even with no name anywhere. Check document headers on anything you attach, since they often carry a setting name you forgot about. Follow your employer's own policy too. Every review includes an anonymity check.
