"Outcome 4 not evidenced"

Nothing in your work points at that outcome, or nothing labels it. Find the outcome wording, find the paragraph closest to it, and make the link explicit. The evidence is often already there and simply unmarked.

"Reflection insufficient" or "go deeper"

Level mismatch

You described where the command word asked you to analyse, evaluate or justify. Common on Level 6 work written at Level 5 depth. Name the alternative you rejected and why — that single move is usually enough.
